Dental Claim Form Backup

When your main office computer is down, or your dental sw program is not working, and you need to get claim forms out, we provide an Excel spreadsheet containing a data entry area where you type raw data, and that information is automatically placed in the appropriate fields of the dental claim form (on the same spreadsheet). The information is also automatically formatted for dates, social security numbers, currency, etc. You can see the data going into the fields as it is entered. When completed, the claim form may be printed for faxing, emailed as an attachment, or snail mailed. The form may be used over and over again.

This form may also be used by those dentists not wishing to pay for expensive dental claim form software. The completed forms may be renamed and saved in patient folders in your computer.
